3.	Examines patients for symptoms or physical information

4.	Orders and executes various test, analyses and diagnostic images to provide information on patients condition

5.	Analyzes reports and findings of tests and examination and diagnoses condition of patient

6.	Administers or prescribes treatments
<br />
7.	Determines and prescribes medication, dosage and schedule given the patients condition and allergies

8.	Discusses any possible side affects to medication or immunization with patient

9.	Prescribes and administers vaccinations to immunize patient from communicable diseases

10.	Promotes health by advising patients about diet, hygiene and methods for prevention of disease

<p>Provides comprehensive medical services for members of family, regardless of age or sex on continuing basis by performing the following duties</p>
<p>Essential Duties/Responsibilities:</p>
<p>1.	Reviews patient file/record, including allergies, problems, medications and immunization status<br />
<br />
2.	Elicits and records information about patients medical history<br />
<br />
3.	Examines patients for symptoms or physical information<br />
<br />
4.	Orders and executes various test, analyses and diagnostic images to provide information on patients condition<br />
<br />
5.	Analyzes reports and findings of tests and examination and diagnoses condition of patient<br />
<br />
6.	Administers or prescribes treatments<br />
<br />
7.	Determines and prescribes medication, dosage and schedule given the patients condition and allergies<br />
<br />
8.	Discusses any possible side affects to medication or immunization with patient<br />
<br />
9.	Prescribes and administers vaccinations to immunize patient from communicable diseases<br />
<br />
10.	Promotes health by advising patients about diet, hygiene and methods for prevention of disease<br />
<br />
11.	Provides prenatal care to pregnant women and provides postnatal care to mother and infants<br />
<br />
12.	Performs surgical procedures and commensurate with surgical competency<br />
<br />
13.	Refers patients to medical specialist for consultant services when necessary for patients well-being<br />
<br />
14.	Documents the patients visit including medical history, physical exam, diagnoses and plan of action<br />
<br />
15.	Follows up with patient regarding progress in high risk or emergency cases<br />
<br />
16.	Conduct physical examinations to provide information needed for admission to school, consideration for jobs or eligibility for insurance coverage</p>

1.	Performs home visits for assessment and evaluation of participants home care needs and develops <br /> homecare plan accordingly.

3.	Documents every home visit and contact and completes weekly Intake and Assessment Report.

4.	Participates in IDTs morning meetings and gives feedback on issues related to home care.

5.	Coordinates information from clinic, day health center and home care staff, and family members in the provision of a homecare plan.

6.	Responsible for the training, supervision and evaluation of homecare staff.

7.	Provides and organizes training for staff and family members in coordination with the Homecare Department.

8.	Assists the Homecare Coordinator in the development and implementation of the initial orientation and on-going education of Homecare staff.

9.	On-site supervision and instruction of homecare workers in participants' home.

10.	Supervises and instructs family/caregivers in care and safety of participants in their homes.

11.	Meets all the administrative requirements including statistical records, medical records, utilizing computerized documentation system, attending staff meetings, etc.

12.	Collaborates with IDT in the development and implementation of plan of care.

13.	Collaborates with Quality Improvement Department in implementing and evaluating quality improvement standards and activities, such as Utilization Review and Quality Improvement.

14.	Participates in multidisciplinary team /family meeting to meet the PACE guidelines and standards in complaints, incidents and grievance.
<br />
15.	Participates in IOA committees, such as Infection Control, Utilization Review, Quality Improvement, and End of Life.

16.	All other reasonably related tasks as assigned.

<strong>EDUCATION &#38; EXPERIENCE:</strong> <br /> <br />
1.	Licensed RN, State of California. Public Health Certificate preferred.

2.	Experience in acute care (at least one year) and homecare (at least 2 years) required. Independence in clinical practice.

3.	Geriatric nursing expertise and preferred palliative care experience.
<br />
4.	Ability to work within the interdisciplinary team with initiative, imagination, resourcefulness and flexibility.

5.	Computer literacy.

- Evaluate minor skin conditions and develop patient treatment plans <br />
- Deliver treatments using established medical protocols <br />
- Actively grow the practice through positive promotion of services offered <br />
- Prepare and participate in online and in-person promotional events <br />
- Write a variety of blog posts on skin conditions, diseases, treatments, and care <br />
- Monitor online dermatology and skincare forums and participate in threads/ conversations <br />
- Assist administrative staff (as needed) with data entry, appointment scheduling, chart reviews, patient check-in and checkout, and filing <br />
- Maintain inventory of medical supplies <br />
- Monitor OSHA policies and guidelines <br />
- Contribute fully to the successful operations of the center and proactively embrace various needs that arise </p>
<p>Requirements: <br />
- Nurse Practitioner License in the state of California <br />
- 2 years experience in clinical environment <br />
- Previous experience in performing laser and injectable treatments preferred <br />
- Previous responsibility for revenue growth of a medical/cosmetic practice preferred <br />
- Excellent interpersonal skills <br />
- Sensitive to confidential material and knowledge of HIPAA/Patient Privacy Practices <br />

PRIMARY DUTIES AND RESPONSIBILITIES: <br />

General Statement of Duties and Responsibilities <br /> <br />
The nurse practitioner is responsible for the triage of adult medicine patients and will supervise an assistant clinic coordinator in establishing a clinical team approach to comprehensive medical care. The assistant clinic coordinator would be charged primarily with the implementation of the team plan for clinic management and would report directly to nurse practitioner triage/clinic manager. This position may allow for up to 20% time continuity clinics in adult medicine or womens clinic and 10% administrative time. <br /> <br />
Reporting Relationships
<br /> <br />
Reports to and is directly responsible to the Chief Operating Officer for administrative and clinic operations functions and reports directly to the Departmental Medical Coordinator in the capacity as a direct medical provider during continuity clinics.
<br /> <br />
Responsibilities <br /> <br />
1.Effectively triages adult medicine patients with appropriate attention to acuity and the availability of medical providers for urgent care evaluation. Medically attends to those patients whose presenting problems are comfortably within employee skill level.
<br />
2.Provides medical treatment as ordered and performs procedures according to MNHC nursing protocol.
<br />
3.Provides leadership, evaluations, and direct supervision to the assistant clinic coordinator and medical assistants.
<br />
4.Supervises the patient flow in clinical area with particular attention to the appropriate triage of adult medicine patients as well as the effective communication with other departments that impact patient flow such as registration, medical records, billing, laboratory, etc. <br />
5.Supervises the preparation of patients for medical evaluation including an assessment that patients are correctly vitalized and monitors availability of charts, labs, and forms.
<br />
6.Supervises the adequate stocking of the patient rooms and the department and efficient re-ordering of materials <br />
7.Supervises the disposition of patients after medical evaluation to ensure that the appropriate medical plans are properly enacted including imparting the relevant health education material.
<br />
8.Supervises the effective participation in Quality Improvement Activities with particular attention to the logs, programs and registries for chronic diseases.
<br />
9.Performs other duties as assigned by Medical Director. <br />
10.Attends and participates in nursing staff meetings.

ESSENTIAL JOB FUNCTIONS:

 Supervise and provide direction for assigned staff: RNs, LVNs and Medical Assistants. 
 Performs nursing duties and works closely with the clinic providers and specialists to provide comprehensive health care to patients. 
 Orients and providers in-service training to staff personnel. Plans and participates in the training of para-professionals and non-medical personnel. 
 Collaborates and coordinates activities with other personnel for the benefits of patient care. 
 Makes appropriate referrals to other departments within the agency as well as to outside community agencies. 
 Uses computerized IT system effectively and documents policy and procedures 
 Involved in risk management and performance improvement with the Medical Director and Administration 
 Develops and is responsible for Nursing schedule coverage to ensure appropriate staffing coverage to match medical providers needs. 
 Attends PAC and Medical Quality Assurance committee meeting, follow-up. 
 Monitor all special programs, such as Homeless, EAPC, HIV, family planning, perinatal, AIM, and BEDP, etc 
 Monitors staffing. Nursing Supervisor always fills in for Nurses and Medical Receptionist. When staff call in sick, sometimes, for more than one staff members. 
 Be prepared to solve any unresolved problems and be troubleshooter. 

QUALIFICATIONS:

 Graduate of accredited school of nursing program. 
 Bachelors degree or two years related worked experience. 
 Minimum of two years recent acute care experience as an RN. 
 Current California Registered Nurse License. 
<strong>Must be Bilingual in English and Chinese.</strong> 
 Demonstrated skills in supervision, leadership, and communication. 
 Compassion to work with the underserved, Chinese community. 
 Willingness to work with para-professional and non-medical personnel. 
 Recent experience in a clinic or out-patient setting desirable.

S/he is responsible for meeting all regulatory documentation standards and collaborative development of procedures to improve implementation of these standards. S/he meets regularly with the nursing staff to provide clinical supervision and facilitates nursing team meetings. S/he participates in the group nursing supervision meetings, multidisciplinary team, and QA and UR activities. S/he acts as a clinical role model and provides nursing consultation to staff within the assigned areas of responsibility. </p><p><strong>Location:</strong><strong> </strong> </p> San Leandro<p><strong>Responsibilities:</strong></p> Provides regular clinical supervision of nursing staff including performance evaluations Reviews and revises procedures and policies to meet regulatory standards Facilitates nursing team meetings Provides training to staff and nurses Participates in multidisciplinary meetings Evaluates illness or injuries. Monitors and implements Physician orders for medication or treatment. Ensures proper storage and procedures for prescriptions in accordance with Community Care Licensing Regulations. Maintains working relationships with Pediatricians, Specialists, Psychiatrists, Therapists and hospitals who provide care to the children. May arrange appointments or receive orders for treatments when nursing expertise is required to facilitate the process. Ongoing training with staff on medications and dispensing medication. Provides other health-related education to staff and children. Maintains medication assistance records. Reviews all medical incident reports for shift. Documents nursing interventions. Participates in multidisciplinary team meetings. Meets regularly with the Program Director and Psychiatrists. Attends appropriate treatment reviews related to health care for children. Is available for questions after hours via an on-call pager. Approves and monitors behavioral restraints Participates in the assessment of client care and documents nursing care needs in the treatment plan Dispenses and records medications.<strong> </strong><strong> </strong><p><strong>Qualifications:</strong> </p> Baccalaureate with 2 years of supervisory experience or Masters with 1 year supervisory experience. Valid California Registered Nurse License Psychiatric interest preferred. POSITION SUMMARY: <br /> <br />

3. Manages minor illnesses, injuries, and the stable, chronic illnesses of participants, according to standard procedures.

4. Evaluates the acute illnesses of the participants.

5. Coordinates appropriate referrals to health care providers in consultation with On Lok physician.

6. Makes nursing home/or home visits when appropriate and as directed by the On Lok physicians.

7. Provides clinical assistance to On Lok's staff registered nurses.

8. Participates in all programs of health restoration, prevention, promotion and maintenance of health.

9. Attends and participates in staff meetings.

10. Fulfills all administrative requirements, including statistical records, chart entries, etc.

REQUIREMENTS:

2. Meet standards of Board of Registered Nursing and the appropriate Business and Professional Code, State of California.

3. Successful completion of a program of study, which conforms to Board of Registered Nursing standards.

4. One year of nursing experience at the RN level and some supervisory experience.
<br /> <br />
5. Willingness to work within the framework of a multidisciplinary team in the On Lok health delivery system.

6. Experience in geriatrics desirable.
<br /> <br />
7. Bilingual ability may be required for specific assignments.

On Lok Inc. pioneered the PACE (Program of All-Inclusive Care for the Elderly) model of care more than 38 years ago; this model has since been replicated at 72 programs in 31 states. On Lok and other PACE sites provide frail seniors with the necessary primary and preventive care, nutrition, prescription drugs, physical therapy, social services and other acute and long-term care participants need in order to live at home rather than in a nursing facility. Participants pay no out-of-pocket costs if they are recipients of Medicare and Medi-cal. <br /> <br />
