RN / RN CASE MANAGERS

PHYSICAL THERAPIST
We have an immediate opening for a Physical Therapist in our South Denver location. We are seeking coverage for the South Denver area.
Full-time as well as Per Diem opportunities are available.
Position Summary:
Reporting to the Director of Clinical Services, the RN Case Manager / Physical Therapists provides professional services to an assigned group of patients and families in the home setting; responsible for on-going coordination and evaluation of care provided by multiple health care disciplines to ensure patients and families are provided information and access to all required services; promotes self-care, prevents episodes of illness and readmission among at-risk clients, when possible; assists patient and caregivers in determining health requirements and resources to remain in the home or for appropriate referral to placement or hospice; and plans and carries out an appropriate course of treatment to meet patient needs.
Minimum employment requirements:
(1) Current Colorado RN / Physical Therapy / LPN license, (2) for RN’s – BSN preferred, (3) minimum 1 year acute care experience and current home care experience required, (4) BLS, (5) current Colorado Drivers License and ability to drive to patientsÂ’ homes, (6) experience with OASIS for Medicare visits.
