The Adult Family Care (AFC) Registered Nurse works collaboratively with the AFC Care Advisor to assess AFC consumers and caregivers and to develop and implement care plans. Conduct initial and ongoing assessments of both AFC consumers and caregivers in collaboration with AFC Care Advisor as required by Mass Health. Prepare care plans for AFC consumers in collaboration with AFC Care Advisor. Monitor consumer needs and update care plans as needed.
Qualifications: Bachelors degree in nursing or related field from an accredited college.
Current valid registered nurse license, issued by the Massachusetts Board of Registration in Nursing. A minimum of 2 years working with seniors and/or individuals with disabilities; preferably within a community setting. Should be comfortable using a laptop or PDA in the field. Personal transportation necessary. Access to automobile and possession of a valid drivers license.
