Clinical Supervisor/Registered Nurse in San Ramon
POSITION SUMMARY:
Responsible for the overall clinical management of a group of patients. Coordinates daily staffing activities of all nursing staff. Responsible for interviewing, hiring, orientation, patient assignments, in-services and evaluation of nursing staff and program.
MAIN DUTIES AND RESPONSIBILITIES (Essential Functions of the Position):
1. Daily oversight and supervision of patients and field staff.
2. Assess and develop an initial plan of care per patients needs in accordance with physician orders and agency policies and procedures. Documentation is appropriate for quality, legal and reimbursement requirements.
3. New and/or changes in plan of care are identified and communicated to physician and clinical staff.
4. Schedules, completes updates and maintains timely employee performance evaluations and competencies
5. On call for patient related problems and available to nursing staff for 24 hour period as assigned.
6. Performs and assists staff in technical nursing procedures according to agency policy as needed.
7. Demonstrates knowledge of State and Federal Regulations and accreditation standards pertaining to home health and is able to communicate this knowledge to staff.
8. Assists staff in utilizing team approach in the provision and reimbursement of patient care.
9. Reports are provided to the Director of Patient Care Services/designee in a timely manner.
10. Assists Staffing Coordinator by calling field staff to cover cases and completes home visits as needed.
EXPERIENCE/TRAINING/EDUCATION/CERTIFICATION:
Required:
Current License as a Registered Nurse
Current BLS-CPR Certification
Minimum of two years experience within the last five years in a home health agency, primary care clinic or health facility.
Sufficient background knowledge and expertise clinical decision-making for the patient population assigned to him or her in the home health agency to meet the needs of his or her patients and to contribute to quality Management review and evaluation.
Current state Drivers License plus proof of car insurance with liability limits per company policy
Home Health supervisory experience preferred.
KNOWLEDGE:
Knowledge of current clinical practices
Knowledge of regulations for home health services, Medicare documentation/OASIS a plus.
